NOFA - Notification Of Funding Availability for OAA Title III-D Services

The Atlanta Regional Commission (ARC), is soliciting proposals from potential applicants interested in and capable of providing a menu of a minimum of the below-listed eight evidence-based health promotion disease and injury prevention program workshops for older adults, adults with disabilities, and/or informal caregivers across the 10-county ARC Planning and Service Area of the following counties: Cherokee, Clayton, Cobb, DeKalb, Douglas, Fayette, Fulton, Gwinnett, Henry, and Rockdale for the period of July 1, 2024, to June 30, 2025. All questions received via email and those asked at the Pre- Proposal Conference will be posted with responses on the ARC website by 5:00 PM EST on May 16, 2024.

3. Chronic Disease Self Management Program
4. Chronic Pain Self Management Program
5. Diabetes Self Management Program
6. Matter of Balance
